Insurance

I am an out-of-network provider and do not accept insurance plans. I am not a Medicare provider. Superbills can be obtained from the patient portal for out-of-network reimbursement. You will need to reach out to your insurance company to determinte your out-of-network benefits. I have partnered with Reimbursify to make filing for out-of-network insurance reimbursements much simpler.

Payments

Patient credit cards or checking accounts are stored in a secure system and are charged after each appointment. Full appointment fees will be charged on the day of the appointment and the patient will be responsible for obtaining reimbursement from insurance.

Cancellation Policy

No-shows and appointments cancelled within 48 hours of the schedule time will be charged in full. 

Patient are encouraged to use the patient portal for refill requests, sharing documents, and checking upcoming appointments. 

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.  Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Medical Board of California

The Open Payments database is a federal tool used to search payments made by drug and device companies to physicians and teaching hospitals. It can be found at https://openpaymentsdata.cms.gov.
